Les documents suivants doivent être fournis le jour du rendez-vous :

1. Formulaire de demande nationale dûment rempli et signé.
2. Une photo d’identité en couleur, fond blanc, datant de moins de 6 mois (voir spécifications).
3. Un passeport valide (et une copie) répondant aux critères suivants :
   1. Il doit contenir au moins deux pages vierges ;
   2. Il doit avoir été délivré au cours des 10 dernières années.
4. Copie d’un titre de séjour français valide.
5. Copie de précédents visas Schengen ou nationaux.
6. Copie du Nulla Osta délivré par le CONI (déclaration nominative d’assentiment).
7. Lettre officielle de l’équipe ou association sportive.
8. Copie du contrat italien.
9. Code fiscal italien (Codice Fiscale).
10. Preuve d’un logement adapté en Italie. Les documents suivants sont acceptés :
    – contrat de location enregistré ;
    – réservation d’hôtel pour une durée d’au moins 30 jours ;
    – déclaration d’hébergement signée par un citoyen italien ou étranger résidant légalement en Italie, conformément aux articles 46 et 47 du DPR 349/1999, confirmant qu’il a mis à disposition du demandeur de visa un logement respectant les normes minimales prévues par la législation régionale relative au logement public.
11. Assurance médicale internationale valable pour la première période de voyage avec une couverture minimale de 30 000 € et une couverture complète et illimitée pour l’hospitalisation. Le rapatriement d’urgence ainsi que le rapatriement des dépouilles mortelles doivent être inclus. Un tableau détaillé des prestations doit être fourni.
12. Réservation de voyage (avion/autocar/train).
13. 116 € en espèces pour les frais de visa.

The following documents must be provided on the day of the appointment:

1. National Application form duly completed and signed.
2. One passport size color photo, white background, maximum 6 months old (see specifications);
3. A valid passport (and a copy) satisfying the following criteria:
   1. it must contain at least two blank pages;
   2. it must have been issued within the previous 10 years;
4. Copy of a valid French permit of stay;
5. Previous Schengen or national visa copy;
6. Copy of the Nulla Osta from CONI (dichiarazione nominativa d’assenso);
7. Official letter from the Sport Team or association;
8. Copy of the Italian contract;
9. Italian fiscal code (Codice Fiscale);
10. Proof of suitable accommodation in Italy. The following documents are accepted:
    – registered rental agreement;
    – Hotel booking of no less than 30 days;
    – Declaration of accommodation signed by an Italian or foreign national legally residing in Italy, in accordance of art, 46 and 47 of DPR 349/1999, confirming that they have made available to the visa applicant a suitable accommodation that comply with the minimum standards laid down in the relevant regional law regarding public housing accommodation;
11. An international medical insurance valid for the first travel period with a minimum of 30.000 € coverage and full and unlimited coverage for hospitalization. Emergency repatriation flight as well as repatriation of mortal remains must be included. Detailed table of benefit must be provided;
12. Travel booking (flight/bus/train);
13. 116€ in cash for the visa fee.
